Terms & Conditions of Use

Information provided through our services does not replace emergency in-person care.

We provide medical services through secure HIPAA-compliant telehealth platforms. By using our services, you acknowledge that:

• Telehealth involves electronic communication
• There are inherent risks, including technical failures
• Telehealth may not be appropriate for all conditions
• You may revoke your consent at any time

To use our services, you must:

• Be at least 18 years of age
• Be physically located in a state where our providers are licensed
• Provide accurate and complete information

📌 Providing false or misleading information may result in termination of services.

When enrolling in a DPC or membership-based program:

• DPC is not health insurance
• Membership does not replace health insurance
• Covered services are limited to those described in your membership agreement
• Hospitalizations and emergencies are not included unless otherwise stated

• Individualized treatments that may not be suitable for all patients
• May include off-label medications
• Require continuous monitoring and follow-up

📌 Results are not guaranteed. Treatment plans may be changed for clinical or safety reasons.

• Medicare patients: we will not charge for Medicare-reimbursable services
• All services must be paid by the patient unless otherwise indicated
• Payment required before services are rendered
• Fees will not change between booking and appointment date

• Subscriptions may be cancelled with 30 days written notice
• Benefits remain active during the notice period
• No pro-rated refunds for partial months

📌 Cancellation requests must be submitted by email or through the patient portal.

Medical services are non-refundable after completion. No-shows or late cancellations may be charged.

• Cancel at least 24 hours in advance
• Late cancellations or no-shows may result in a fee
• Repeated no-shows may result in termination of care

We reserve the right to refuse or terminate services if you violate these Terms, fail to comply with treatment plans, provide false information, or continued treatment is no longer clinically appropriate.

📌 Termination does not relieve you of financial responsibility for services already rendered.

Use of our services is subject to our Privacy Policy and Notice of Privacy Practices, in compliance with HIPAA.

You agree not to misuse the site or patient portal, not to attempt unauthorized access, and not to disrupt the functionality of the website.

To the maximum extent permitted by law, Clínica Geral USA shall not be liable for indirect or consequential damages arising from the use of our website.

These Terms are governed by the laws of the State of Colorado.

We may update these Terms at any time. Continued use of services constitutes acceptance of the updated Terms.
